1. Stunt or aerial work,
2. Influence of alcohol or narcotics,
3 Insured person’s Participation in any brawl,
4 Riots & Civil Commotion
Procedure of Claims:
- Visit any or your family doctor immediately after you injury during a hockey match.
- If you need to see physio, you need to ask your family doctor to get a refer letter.
- Make copies of all receipt and keep the original as well, call your captains for claims form.
- Complete the form and send back to Shaheen Sports Club, c/o Room 305, 8 Jordan Road, Kowloon.
